Resumen

Objetivo: Evaluar la adherencia, el porcentaje de embarazos y el perfil de efectos adversos reportado por las pacientes a los seis meses y al año posterior a la colocación del sistema liberador de levonorgestrel o implante subdérmico de etonogestrel de las pacientes adolescentes del Servicio de Ginecoobstetricia del Hospital Rafael Ángel Calderón Guardia entre el periodo de junio 2017 a diciembre 2018. Método: Estudio descriptivo, transversal, observacional y retrospectivo de registros médicos. Analiza los datos de adolescentes (mujeres menores de 20 años de edad) del Servicio de Ginecoobstetricia del Hospital Calderón Guardia, a las que se les colocó algún LARC entre enero 2017 y diciembre 2018. Se evaluaron 114 pacientes con información suficiente para el análisis de la respuesta de los anticonceptivos a los 6 meses y al año de colocado. Se utilizó una hoja de recolección de datos, sin documentar la identificación de la paciente, con información sobre el momento de la colocación del anticonceptivo, además de los datos de las variables a estudiar documentadas en el expediente a los 6 meses y al año de colocación del LARC. Los datos recolectados a una tabla de análisis de Windows Excel, mediante la cual se realizó el análisis estadístico. Se realizó un análisis descriptivo de las variables en estudio, representando el número y porcentaje de pacientes. Se analizaron las variables cualitativas y cuantitativas. Se utilizaron cuadros y tablas para describir los resultados obtenidos. Resultados: Se analizó la efectividad, adherencia, satisfacción y los efectos adversos reportados por las pacientes asociado a cada método anticonceptivo. Durante el periodo de estudio de dos años se colocaron 48 sistemas intrauterinos y 331 implantes subdérmicos. Se les dio seguimiento a los 6 meses y al año a 16 sistemas intrauterinos y a 98 implantes subdérmicos. La efectividad del método en las pacientes analizadas al año de uso fue de un 100%, con una adherencia de 92,1%, mientras que únicamente un 7,1% de las pacientes se retiraron el método anticipadamente. La razón de abandono fue en su mayoría debido a efectos adversos reportados por las pacientes. Del total de pacientes un 35,4% no presentaron ningún efecto adverso. De las pacientes que sí reportaron alguna molestia, en mayor frecuencia se documentó sangrado irregular (en un 31,5%) al año del uso del método. Otros efectos adversos encontrados en menor frecuencia fueron cefalea, aumento de peso, sangrado abundante, mareos, dolor en sitio del implante, acné, ansiedad, depresión y dismenorrea. Conclusión: El porcentaje de falla (de embarazo) terapéutica con cada uno de los métodos anticonceptivos al año de uso fue de 0%. El porcentaje de retiro anticipado del método fue de un 7,1%, mientras que un 92,9% de pacientes continúan usando el anticonceptivo al año de uso. Los principales efectos adversos con el uso de métodos anticonceptivos de larga duración fueron sangrado uterino irregular y cefalea a los 6 meses y al año de uso, tanto para el implante subdérmico como para el sistema intrauterino. La principal razón de abandono del método anticonceptivo fue debido a efectos adversos, siendo el más frecuente sangrado irregular.


Abstract

Objective: To evaluate adherence, the percentage of pregnancies and the profile of adverse effects reported by patients at six months and one year after placement of the levonorgestrel-releasing system or etonogestrel subdermal implant in adolescent patients of the Gynecology and Obstetrics Service of the Hospital Rafael Ángel Calderón Guardia between June 2017 and December 2018. Methods: Descriptive, cross-sectional, observational and retrospective study of medical records. It analyzes the data of adolescents (women under 20 years of age) of the Gynecobstetrics Service of the Calderón Guardia Hospital, who had any LARC placed between January 2017 and December 2018. We evaluated 114 patients with sufficient information for the analysis of contraceptive response at 6 months and 1 year after placement. A data collection sheet was used, without documenting patient identification, with information on the time of contraceptive placement, in addition to data on the variables to be studied documented in the record at 6 months and at 1 year of LARC placement. The data collected were entered into a Windows Excel analysis table, which was used to perform the statistical analysis. A descriptive analysis of the variables under study was performed, representing the number and percentage of patients. Qualitative and quantitative variables were analyzed. Tables and charts were used to describe the results obtained. Results: The effectiveness, adherence, satisfaction and adverse effects reported by the patients associated with each contraceptive method were analyzed. During the two-year study period, 48 intrauterine systems and 331 subdermal implants were inserted. Sixteen intrauterine systems and 98 subdermal implants were followed up at 6 months and 1 year. The effectiveness of the method in the patients analyzed at one year of use was 100%, with an adherence rate of 92.1%, while only 7.1% of the patients withdrew the method early. The reason for withdrawal was mostly due to adverse effects reported by the patients. Of the total number of patients, 35.4% did not report any adverse effects. Of the patients who did report some discomfort, irregular bleeding (31.5%) was most frequently documented one year after use of the method. Other adverse effects found less frequently were headache, weight gain, heavy bleeding, dizziness, pain at the implant site, acne, anxiety, depression and dysmenorrhea. Conclusion: The percentage of therapeutic failure (pregnancy) with each of the contraceptive methods at one year of use was 0%. The percentage of early withdrawal of the method was 7.1%, while 92.9% of patients continue to use the contraceptive at one year of use. The main adverse effects with the use of long-acting contraceptive methods were irregular uterine bleeding and headache at 6 months and 1 year of use, both for the subdermal implant and the intrauterine system. The main reason for abandonment of the contraceptive method was due to adverse effects, the most frequent being irregular bleeding.
